Do not send in any of these forms at this time. Go to the 'Apply' page to fill out and submit the application form. Completing a class application form is a step toward final registration, but it does not guarantee you a place in any class. Class application forms will be randomly selected in instances where demand exceeds capacity. If we are not able to accommodate your requests, we will contact you to help you find another time or location, if possible. The forms you download from this page will be collected on Site Registration Day.
